Q: What information should I bring to my first visit?
A: Please bring your registration papers if not previously mailed. Also bring your insurance card(s) and any Worker’s Compensation information that may apply to this appointment. If your insurance plan requires a copay, we will be collecting this at the time of your visit. We accept cash, checks, Visa, MasterCard, Discover and American Express. Please check to see if your insurance plan requires a different copay amount to see a specialist. Fox Valley Surgical Specialists is a specialty surgical clinic.
Q: Who do I contact regarding questions related to my office visit?
A: Each of our physicians has a professional Medical Assistant who will be your primary contact regarding questions and/or concerns regarding your appointment and plan of care.
Q: If I need to cancel my office appointment, who do I contact?
A: If you need to cancel your appointment, please contact FVSS at 920.731.8131 or 800.574.3872. For the convenience of other patients, please provide a 24-hour notice when you find it necessary to change your appointment. If you need to cancel a scheduled procedure, please ask for your physician’s Medical Assistant.
Q: What should I do in case of severely inclement weather?
A: To ensure your safety and the safety of our staff, please call our office to make certain that our planned schedule has not changed. If you cannot travel safely, please contact the FVSS office at 920.731.8131 or 800.574.3872 and we will reschedule your appointment.
